Children 12 and under: Ear piercings only. Parent must have photo ID and child birth certificate.
Children 13 and over: Parent must be present and have their photo ID. Minor must have Photo ID and Secondary ID (no exceptions).
Acceptable Photo IDs for minors: Drivers license, state photo ID, yearbook, printed official photo with name.
Acceptable ID to prove signer is parent or guardian: Birth certificate with parents name listed, insurance card with both parent and child names, state guardianship paper with raised seal, mail addressed to minor at parents address.
